Freedom of Information Requests

The circumstances of your loved one's death determine which reports are generated by the work of the Coroner. A completed Freedom of Information Request form (PDF) is required for all report requests. You may complete the electronic form below; or download/print a Freedom of Information (FOI) Request form; or you may obtain a FOI form by emailing the Coroner's Office or calling 309-669-2000 between 8:30 a.m. and 4 p.m. Monday through Friday (except on County holidays). Once payment is received for requested copies, your Freedom of Information request will be completed within five business days whenever possible. If there is an inquest, no copies of reports will be released until the time of inquest.

Fees

The statutory fees for a copy of reports is determined by State Statute 55 ILCS 5/4-7001, revised in July 2010, by SB2529. Prepayment is required for obtaining copies of reports. Accepted forms of payment are cash, credit card, money order, cashier's check, or personal check made payable to Office of the Peoria County Coroner. The copy fees are as follows:

  • Autopsy Report: $50
  • Investigation Report: $25
  • Inquest Transcript: $5 per page
  • Inquest Verdict: $5 
  • Toxicology Report: $25

The general public may obtain a copy of the transcript and verdict of an inquest after paying the statutory fees, as an inquest is a public inquiry. All other available coroner's records pertaining to your loved one may be obtained only by the immediate family, attorney, or private business representing the family or your loved one. The Peoria County State's Attorney's Office will be consulted before releasing copies of reports generated by the work of the Coroner when the death involves criminal circumstance.
